An institution-specific audit framework
A scoped work program, evaluation criteria, evidence requests, interview guides, test procedures, and findings and closure templates. Includes agreed induction and interpretation support.
Commission an institution-specific audit framework, a Phil-led independent review, or neutral interviews with financial institution and vendor personnel. Findings connect operational evidence to reporting, controls, and the decisions leadership needs to make.

An independent audit should establish whether delivery, reporting, and controls support the decisions leadership is being asked to make. The review examines the agreed engagement or portfolio subset against applicable institutional, contractual, and control criteria.
Operational records, work samples, reporting definitions, and direct accounts from relevant bank and vendor personnel are assessed together. Findings distinguish observed facts, corroborated accounts, estimates, disputed evidence, and material gaps.
The review can support continuation without further Akholi work. It does not presume that the weaknesses reported in the study will recur in the institution’s engagements.
A scoped work program, evaluation criteria, evidence requests, interview guides, test procedures, and findings and closure templates. Includes agreed induction and interpretation support.
Phil directs the review, coordinates agreed institutional and specialist resources, challenges evidence, and delivers supported findings and the executive decision paper.
A defined interview and evidence module for a commissioned review. Covers participant selection, confidentiality limits, corroboration, reporting, and escalation of urgent concerns.
The audit is a stand-alone purchase. Fees are not contingent on adverse findings or the sale of recovery work. Relevant relationships and financial interests are disclosed before acceptance. An external interview role must not simultaneously advocate for a party under review.
If Akholi is later commissioned to lead management corrections, that work has a separate mandate. Qualified personnel who did not implement the corrections provide independent verification of material closure. Explore Engagement Recovery.
The service puts the Executive Action Plan’s independent review into practice through a scoped institutional mandate. The study recommends a role and a program of work. It does not appoint an exclusive provider or create a regulatory certification.
Yes. Commission a framework adapted to the institution and engagement, with agreed internal use, induction, interpretation, and update arrangements.
The assignment can begin with an inventory and risk prioritization, followed by detailed reviews of selected engagements. Review depth, resources, scope limitations, and timetable are agreed. Portfolio triage is not represented as a detailed audit of every engagement.
Agree access to identifiable records, privacy, participation arrangements, escalation, and limits of confidentiality before interviews begin. Accounts are assessed alongside operational evidence and contrary information.
The decision, engagement complexity, review period, evidence availability, interview base, access rights, institutional resources, and specialist requirements. Scope, timing, and fees are agreed privately.
Phil Hatch’s 2026 study examines 150 agentic AI outsourcing engagements through accounts from 850 vendor delivery-team respondents. It identifies reported weaknesses in preparation, shared management, controls, reporting, and the visibility of human work.
At least one form of manual alteration was reported on all 150 engagements. Written accounts classified as governance weaknesses also occurred on all 150. These findings support examination of actual delivery and control operation. They do not establish failure of every control.
The Executive Action Plan proposes independent review, direct access to delivery personnel, corrective action, and independent verification of material closure. Akholi translates those recommendations into a scoped institutional assignment.
Related evidence: financial institution readiness, co-management, and manual alteration.
The sample is not industry-representative. The findings are observational. They do not establish causation or legal breach, and the study did not test the proposed remedies or measure GCC performance.
